2. Submit the SSO 9-02 Hospital Selection Form at any Social Security Office in Bangkok, province, or branch nationwide. • Complete and sign the SSO 9-02 form https://www.sso.go.th/wpr/ • Copy of your national ID card with your signature.
3. Change your Social Security rights via the SSO Connect app. • Download http://onelink.to/ae83ed?&dev=macos&ref= • Register with your national ID card. • Wait for your identity to be verified via OTP on your mobile phone. • Select the “Change Hospital” menu in the app. • Select the desired hospital and press “Confirm.” • Wait for the change to be verified within 2-3 business days.
